MINDDOG ANNUAL ASSESSMENT
The Public Access Test (PAT) is an annual assessment of mindDog’s suitability as an assistance dog accessing public spaces.
This test is used to assess a dogs suitability within the public domain. The PDF below details the requirements dogs need in order to pass the Public Access Test.
FIRST VISIT
The form that the assessor will fill in when they first visit you
3 MONTH TRAINING RECORDS
The form that you will fill in to show how you are progressing after 3 months
6 MONTH TRAINING RECORDS
The form that you will fill in to show how you are progressing after 6 months
SECOND VISIT
The form that the assessor will fill in when they review your training progress
9 MONTH TRAINING RECORDS
The form that you will fill in to show how you are progressing after 9 months
PUBLIC ACCESS TEST
The form that the assessor will complete when they assess you during the Public Access Test
